Nursing Facility Pre-Admissions Screening
Indiana's Nursing Facility Pre-Admission Screening (PAS) program can help you evaluate your situation and provide information on possible alternatives to admissionto nursing facility admission.

PAS is a process consisting of an application, a comprehensive assessment of needs, a plan of care, and a finding or decision concerning the most appropriate placement for an individual.
The application will initiate an appointment with a case manager. The case manager meets with the individuals and the family to discuss any medical problems, how much help is needed with activities of daily living, and the kinds of help that may be provided in the home. The case manager will also obtain information from the individual's physician.
Based on the assessment and knowledge of alternative services available in the area, the case manager recommends either nursing facility care or continued living at home with supportive services.
If you would like to learn more about these services, contact NWICA 2-1-1 Information and Assistance at 2-1-1 or 1-800-826-7871.
